In the final episode, Cha Eun-hwan (Kim Myung-soo) and Yu Ji-an (Kang Min-ah) reunited after one year and faced the truth of a past accident. Yu Ji-an learned that Cha Eun-hwan's father lost his life while trying to save her in the accident that changed her childhood, and that Cha Eun-hwan's younger sibling Cha Song-hwan (Cha Min-ji) began using a wheelchair as a result of the incident, leading her to deeply understand Cha Eun-hwan's pain.
Meanwhile, Choi Jeong-mi (Son Ji-na) returned to Korea and took over Yu Ji-an's management, intervening in the film production process. Choi Jeong-mi pressured to reduce the screen time of Han I-jin (Kwon So-hyun), which placed a heavy psychological burden on Yu Ji-an. Eventually, Yu Ji-an became embroiled in a power trip controversy and went into hiding. Cha Eun-hwan struggled to find Yu Ji-an, and Cha Song-hwan suffered as she recalled the moment she had asked Yu Ji-an to leave.
In the end, Cha Eun-hwan tracked down Yu Ji-an's location and reunited with her. Yu Ji-an told Kim Jun-sik (Jung Hee-tae) that her mother Choi Jeong-mi had orchestrated the power trip controversy, and declared she would sever ties with her. She then delivered a contract termination notice directly to Choi Jeong-mi, saying, 'I will no longer ruin myself just to be loved by my mother.'
The two visited the columbarium where Cha Eun-hwan's father's remains are kept. Yu Ji-an confessed that she had known all along that Cha Eun-hwan's father had passed away because of her, and bid him farewell.
One year later, Cha Eun-hwan discovered a bouquet of flowers at the columbarium. Upon stepping outside, he came face to face with Yu Ji-an in a nearby flower field, and the two reunited with smiles on their faces.
The drama 'Empathy Cells' reminded viewers once again of the true meaning of empathy and communication through its theme and narrative of emotional conveyance.
